New Patients

Welcome to Psychiatry and Wellness of Reno!

Thank you  for choosing Psychiatry and Wellness of Reno.

 

We see patients by appointment only.  

 

Please complete the New Patient Intake Form (click for link to form).  If you plan to use insurance, please make sure that we take your insurance (click link to view insurance plans).

The process to make an appointment usually takes 1 week and will follow these steps:

  1. One of the doctors will review your responses to make sure that our practice is a good fit for you.

  2. Our staff will verify your insurance coverage and contact you to discuss deductibles, co-insurance, or other billing topics.

  3. Our staff will contact you to make an appointment.

 

Your first appointment will be 50 minutes, and follow-up appointments are 25 minutes.

You and the doctor will determine how often appointments will be scheduled.  

Currently, new patient appointments are running 2-3 months out.  If we have cancellations, appointments can be adjusted forwards.

Please bring in the following to your first appointment:

  • Driver's license

  • Insurance card

  • Updated credit card to put on file

  • Copayment / Co-insurance

  • List of current medications

  • Signed Release of Information for your Primary Care Provider, previous psychiatrist, therapist, psychiatric hospital if applicable

Your first intake appointment will be 50 minutes.  Follow-up appointments are typically 25 minutes.  Please arrive 15 minutes prior to your appointment time.  If you arrive late to an appointment, we will make every effort to accommodate you; however, we can not guarantee that you will be seen.  We ask that you give at least 48 hours notice if you cannot make an appointment allowing us to schedule in emergency appointments.  All appointments not cancelled more than 48 hours prior to your scheduled time will be charged a fee ($300 for intake appointment and $150 for follow up appointments).  All accounts must be up to date to schedule future appointments and to make refill requests.
